SUPPORT BROKER SUMMARY:

Non Profit Care Coordination, Inc. (NPCCI) has contracted with the Department of Mental Retardation (DMR) to further expand the ability of adults with developmental disabilities to direct their DMR provided resources by selecting the supports that individually meet their own needs, rather than having DMR assign those resources. This is part of a nation wide movement originated by families and advocates to empower people with disabilities to make their own life choices.

The Support Broker position is part of a pilot program to further this movement across the state of Massachusetts. Support Brokers will work both one-on-one and in groups with the developmentally delayed individuals and their families who choose to participate in this program by helping guide them through the self-determination process and teach them the key aspects of person-centered planning. This position offers part and full time options depending on the case load and hours per case. It is expected that a case would range from 2-24 hours per quarter.

OVERALL RESPONSIBILITIES:

Assists individuals in planning, developing and directing their own supports; Provide person-centered planning for individuals; Provide oversight of services by maintaining regular and consistent contact with individuals & guardians; Monitor the safety and well being of individuals; Facilitate linkage between the Individual Support Planning (ISP) process and the supports and services to be provided, purchased or arranged; Assist individuals to recruit, hire, and train staff; Participate as needed in providing safeguards and quality assurance oversight; Advocate for individuals’ rights; Inform DMR on abuse and neglect; Facilitate and assist with an individual’s benefits and individual budgeting; Qualify staff and arrange service agreements between individuals and providers; Respond to requests for information and data verification from the DMR Service Coordinator and others.

DUTIES:

Person Centered Planning

  • Provide education and information about self direction
  • Assist participants with initial enrollment by reviewing of roles and responsibilities
  • Assistance with explanations of tax issues and employer of record if individuals hire staff directly
  • Facilitate a formal meeting process to assist in identifying goals, needs and services
  • Facilitate one-on-one sessions with individuals & family around planning making changes if desired

Fiscal Management

  • Assist individuals in budget planning in collaboration with DMR staff for initial budget
  • Assist individuals in making on-going changes to budget
  • Problem solve payment issues if they arise on behalf of the individual.
  • Assist individuals with negotiation of rates with service providers

Resource Development

  • Assist individuals in recruiting, interviewing, hiring & firing staff
  • Arranging for training of staff and service agreements
  • Assist participant in supervision of staff
  • Assist individual in overall oversight of staff
  • Development of other resources: classes, services, housing, goods that are non traditional
  • Ensure new staff are properly qualified and/or certified to provide supports required

Quality Assurance

  • Incident reporting
  • Establish a back-up service plan
  • Provide on-going oversight of the back-up service, safety, emergency plans
  • Verification that services are being provided
  • Monitoring of fiscal report

Coordination & Communication with DMR Staff

  • Attendance at Individual Service Plan with DMR
  • Work with DMR Area Office on budget issues
  • Work with DMR Service Coordinator around individual’s needs

Preferred Qualifications:

  • Prior supervisory experience and both administratively and clinically demonstrated program development skills
  • In depth knowledge of Self Direction and Person Centered Planning concepts.

Required Qualifications:

  • Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, or physical
  • At least three years of professional experience in human services.
  • The pilot pool of developmentally delayed individuals is located in Boston, Metro Boston Communities and North Shore, with possible expansion statewide. Based on assignment, travel is required. Applicants who elect to use a motor vehicle for travel must have a current and valid Massachusetts Class D Motor Vehicle Operator's license or the equivalent from another state.

SUBSTITUTIONS:

  • Education may be substituted for two years of the required experience on the basis of two years of education for one year of experience.
  • One year of education equals 30 semester hours. Education toward a degree will be prorated on the basis of the proportion of the requirements actually completed.
  • No substitutions will be permitted for the required year of professional experience.
